These are to be reviewed in accordance with policies and frameworks (internal and external)Participating in programs that may enhance the older persons wellbeing, quality of life and preference to remain living in their own homeConduct and complete comprehensive assessments and defined measures, and use specified data collection tools and outcome measures to record and demonstrate changes in function and to ensure consumer outcomesAbout you?
