Community Care Lead - Mid Coast
The Opportunity:
We have the perfect opportunity for a registered nurse or enrolled nurse who is client focused, has experience in aged care and would like to move into a role where you can maintain your clinical skills without the shift work.
Joining our Community Team as a Community Care Lead, you will oversee the daily operations of the Community Aged Care team in the Mid Coast region in relation to the provision of appropriate aged care services to align with the relevant funding agreements and in line with the Aged Care Quality Standards, Charter and relevant organisational policies and procedures.
Team overview
Reporting to the Commercial Director Community & Allied Health, you will lead a passionate team of 9 home support staff and ensure clients are receiving the full breadth of support they need to stay in their home and achieve great outcomes.
We are open to this position being part time or full time for the right candidate with hybrid working options and office work space available.

Role & Responsibilities:
- Provide a point of liaison, guidance and reference for consumers, carers, staff and managers with regards to any matters related to STRC / CHSP / HCP Home Care
- Coordinating care and support services for consumers accessing home care services in accordance with the Aged Care Quality Standards and within the funding arrangements of the HCP, CHSP and STRC.
- Organise and attend meetings relating to Community Care Services as applicable to the role
- Maintaining My Aged Care records for all referrals, upgrades and consumer services as relevant for delegated services.
- Managing delegated referrals and commencement of new consumers and services
- Manage employee performance for all direct reports, providing regular feedback to employee and conducting annual performance reviews.
- In collaboration with management, ensure that the Home Care budget is being managed correctly and as such appropriate actions occur to ensure the budget operates within the parameters set by Finance
About you?
- Registered Nurse with current Registration under AHPRA (Australian Health Practitioners Regulation Agency).
- Would consider enrolled nurse with 5+ years experience including management experience (min 2 years)
- Experience in the Health, Aged Care or Home Services Sectors or a demonstrated comprehensive understanding of these sectors.
- Experience in client assessment, care planning and documentation.
- Proven team leader.
- Understanding of the Aged Care Standards and Home Package Care Guidelines.
- Comprehensive experience in working in a flexible, multi-skilled team environment with advanced interpersonal, communication, customer service and networking skills.
