The Opportunity:An exciting opportunity for an experienced and passionate Enrolled Nurse to join Anglican care Home Care, Community. You will be responsible for assessing and coordinating care and support service to our consumers who still live their own home.Along with the wider Home Care team, you will provide support, education and resources to consumers so they can make informed decisions and have choice and control over the services and support they receive and ensure the services delivered are safe, effective and meet the consumers' needs. The Care Coordinator role also incorporates providing clinical care within your scope of practice, which is based on your training and education, skills and experience.Why work for us?
