This role offers an excellent opportunity for enthusiastic and energetic individuals looking to make every moment matter by bringing respect, compassion, integrity and enhancing the wellbeing of our residents and consumers.Essential:Registered Nurse with post graduate qualifications as a Nurse Practitioner and eligible to apply for endorsement with the Australian Health Practitioner Regulation Authority (AHPRA).Demonstrated evidence of recent Professional Development in the areas of Palliative and Aged Care with extensive skills and knowledge in advance care planning, the palliative approach in aged care, end of life care for older people facing life limiting illness and Gerontology.Proven record of ability to build community relationships, network and provide leadership in the area of gerontology and palliative aged care.Demonstrated knowledge and understanding of Commonwealth and State legislation, and Policy, governing the provision of aged care services.Demonstrated knowledge and understanding of palliative care services and other support services available to older people; and their families, facing life limiting illness.Provision of evidence of the development of resources to support education and relevant information for consumers, carers and professionals.Extensive clinical experience working within residential and community aged care with consumers living with advanced chronic disease and frailty.Demonstrated ability to work as a change agent in relation to clinical practice.Superior written and verbal communication, negotiation and interpersonal skills.Current First Aid Certificate (ongoing requirement)Desirable:Proficiency in the use of Microsoft Word, PowerPoint, Excel and Publisher.Current NSW Drivers Licence.At Anglican Care our skilled and caring people are the most important resources we have.
