As a Case Coordinator you will have the opportunity to do this each and every day, bringing respect, compassion, integrity and to support people to live independently at home for as long as possible.The team are seeking Case Coordinators with a genuine passion for working with the elderly.About the opportunityOur Case Coordinators are responsible for coordinating care and support services for people accessing Home Care Packages with Anglican Care. We are looking for enthusiastic and motivated people who can provide support, education and resources to our consumers so they can make informed decisions, and have choice and control over their services and the support they receive.This role will incorporate conducting comprehensive assessments and developing goal oriented support and care plans in consultation with consumers and/or significant others and in collaboration with other health and care professionals.By facilitating pro-active case management processes to include individual consumer and/or therapy plans and conducting comprehensive assessments, developing goal oriented support in consultation with consumers and/or significant others and in collaboration with other health and care professionals, our aim is promote consumer engagement and self-determination to achieve optimum quality of life within their own home. Essential requirementsMinimum Certificate IV and relevant knowledge and experience in aged care, health or disability; or tertiary qualifications in relevant health related disciplineExperience in case management/ care coordinationUnderstanding of Home Care Packages and the Aged Care Quality StandardsUnrestricted Drivers LicenceEvidence of COVID-19 vaccinations, or willingness to obtain prior to commencement (unless medically exempt)Desirable skills/knowledgeSound knowledge and practical experience working with people who are ageing from an enablement, restorative and wellbeing perspectiveExperience working with people who have dementia and their carersExperience and commitment to working with quality and risk management systemsCapacity to provide education and hands on learning in the workplaceProven ability work as part of a team and foster a positive workplace culture; or experience working as part of a multidisciplinary team At Anglican Care our skilled and caring people are the most important resources we have.
