The Opportunity: An exciting opportunity for a Case Coordinator to join our Community team on the Central Coast. You will be responsible for coordinating care and support services for consumers accessing Home Care Packages in accordance with the Home Care Standards and within the funding arrangements of the HCP and CHSP.The purpose of this position is to provide support, education and resources to consumers so they can make informed decisions and have choice and control over the services and support they receive and ensure the services delivered are safe, effective and meet the consumer’s needs.This is a Part Time Permanent vacancy.Why Work for us? Role & Responsibilities: Function as a case adviser in relation to the provision of home support and home care packages. Ensure the alignment of technologies, methods and practices with the Home Care Standards, Charter and relevant organisational policies and proceduresIdentify individual consumer’s needs and implement a care plan and budget in consultation with the consumer, carer and relevant stakeholders (internal and external)Ensure care plans are developed based on the goals, strengths and identified needs in consultation with consumersProcessing package upgrades and transfers as directed, including developing care plans, budgets, Agreements and consentsMaintaining My Aged Care records for all referrals, upgrades and consumer services as relevant for delegated servicesProvide a point of liaison, guidance and reference for consumers, carers, staff and managers with regards to any matters related to Home CareAssist with planning, implementing, monitoring and reviewing all aspects of care and support in consultation with consumers, in a timely and professional mannerEnsure individual consumer budgets are managed and monitored within financial parametersAbout you?
