As a crucial part of our Home Care Service team, you will be responsible for the assessment and facilitation of services for our clients to improve their quality of life.Your role will include assessment, ongoing education, rehabilitation, interventions such as home modifications and equipment prescription to support clients in their home or in our day therapy centres . This role is suited to a high functioning clinician who can work as part of a multidisciplinary team and also work autonomously.About youYou are a team player with a high level of energy and enthusiasm that thrives on helping others.
