As a Clinical Nurse Practitioner you will have the opportunity to do this each and everyday, bringing respect, compassion, integrity and enhancing the wellbeing our residents and consumers.Located within our Kilpatrick Court Aged Care home, the team at Anglican Care Residential Care Services are seeking an experienced and professional Clinical Nurse Practitioner who will be responsible for the provision of expert clinical care and decision making that leads to person centred, end of life experience, for residents with advanced life limiting chronic disease including dementia.The successful applicant will have the ability to develop key relationships with Residential Care Managers, GP’s and other health professionals to ensure a collaborative and consensus building approach to end of life care planning, supporting self-determination and promoting person and relationship centred care. Your strong interpersonal, communication and negotiation skills will prove invaluable as you work collaboratively with a range of staff.To be able to apply for this role you will have:Essential:Registered Nurse with post graduate qualifications as a Nurse Practitioner and eligible to apply for endorsement with the Australian Health Practitioner Regulation Authority (AHPRA).Demonstrated evidence of recent Professional Development in the areas of Palliative and Aged Care with extensive skills and knowledge in advance care planning, the palliative approach in aged care and end of life care for older people facing life limiting illness.Proven record of ability to build community relationships, network and provide leadership in the area of palliative aged care.Demonstrated knowledge and understanding of Commonwealth and State legislation, and Policy, governing the provision of aged care services.Demonstrated knowledge and understanding of palliative care services and other support services available to older people; and their families, facing life limiting illness.Provision of evidence of the development of resources to support education and relevant information for residents, carers and professionals.Extensive clinical experience working within residential and community aged care with residents/clients living with advanced chronic disease and frailty.Proven record of experience in project management and reporting against a Commonwealth evaluation strategy including experience in the development and review of policy, clinical guidelines and clinical pathways/frameworks.Demonstrated ability to work as a change agent in relation to clinical practice.Superior written and verbal communication, negotiation and interpersonal skillsCurrent First Aid Certificate (ongoing requirement)Desirable:Proficiency in the use of Microsoft Word, PowerPoint, Excel and Publisher.Current NSW Drivers Licence.At Anglican Care our skilled and caring people are the most important resources we have.
