The purpose of the Clinical Care Coordinator role is to ensure the overall clinical management of a designated number of consumers in line with the Aged Care Quality Standards, and other relevant legislation, to ensure Anglican Care meet the needs of their consumers. This is a great step for a Registered Nurse with experience in an Aged Care environment. Please see attached position description and the essential and desirable criteria below: Essential:Registered Nurse with current Registration under the NSW Nurses Registration Board.Capable of leading by example and being a role model to all staff in living these Values.High level competence in the provision of Nursing and clinical skills within an Aged Care environment.Demonstrated knowledge of the Health and/or Aged Care SectorsDemonstrated high level skills in Aged Care Assessments, recognising and responding to acute and chronic changes, and communicating plan of care with consumers, their families and carers, and relevant employeesDemonstrated competence and understanding of aged care funding. Desirable:Demonstrated competence in: Leadership; Operational Planning; Human Resources; External Relations, Project Management, Time Management and Service Provision.Comprehensive experience in working in a flexible, multi-skilled team environment with advanced interpersonal, communication and customer service skills.Demonstrated knowledge of funding and policy protocols, accreditation and industrial legislation.Tertiary qualifications in Geriatric Nursing or demonstrated competence in that field or working toward same.Computer literacy and documentation skills.Aged Care is in high demand.
