This important role brings compassion, respect and support to our seniors. The successful applicant will have a high standard of clinical and case management skills to provide the best possible care and customer service for our consumers. The role enables you to work with flexibility and autonomy, dividing your time on the road visiting our consumers, liaising and consulting with our internal team of Nurse Practitioners, Clinical Nurse Consultants, Clinical Nurse Specialists and Allied Health Professionals, as well as networking with relevant community stakeholders, to ensure that we make a difference to the lives of our consumers.
