What is HomeLife?

We’re here to help you live independently in your home for as long as possible.

HomeLife provides personalised support across lifestyle, exercise and wellness, health and nutrition, personal care and help around the home.

The Australian Government funds a number of different options to support seniors who want to maintain their independence and live in their own home, including Support at Home and Commonwealth Home Support Program (CHSP).

Our personalised Home Care Packages provide coordinated care and support to ensure you continue to lead a good life while living at home.

Firstly, one of our HomeLife Care Partners will sit down with you to help clarify your specific goals and needs. This is an important step in getting to know you (and you getting to know us) so that we can provide the best possible care and support.

Then we’ll develop a care plan together that’s as individual as you are, taking into account everything that matters most to you.

We can help you with services that cover lifestyle, exercise and wellness, health and nutrition, personal care and help around the home.

Bringing your home care to you.

Our Home Support services bring support into your home. Whether you need help around the home, or a higher level of care, our Home Support services will meet you where you are.

Our Home Support can provide the following services through Support at Home funding:
We bring your clinical care to you and ensure you have access to a range of allied health professionals and clinical care providers such as nursing and physiotherapy.
These services will give you the confidence to stay at home for longer and will help you with personal care and mobility support.
Your home should be comfortable and meet your needs. To do so, we provide services to support you in your daily life, from cleaning and housework to lawn and garden care.

Day Support Centres

For individuals that need a little extra support to stay active and social.

When attending our Day Support Centres, you will spend the day enjoying a variety of wellbeing activities of your choice, all while being supported by our qualified team of home care staff and nurses in one of our secure, purpose-built centres. We are here to support you to have fun, be well and be you.

How to Apply

Ready to apply for HomeLife?

To get started you will need to follow the steps below.

Remember our Customer Engagement Team are always here to give you or your family support through the process.

If you need further support please get in touch.
To access these services, you will first need to apply for funding through the Australian Government’s service, My Aged Care. They will organise an assessment and advise on your funding eligibility.
Choosing a provider is a very personal decision, you want to feel comfortable with the quality of the service and support you receive. We believe that our highly skilled team are motivated by genuine compassion, and that’s what sets us apart from others.
Once your funding has been approved by My Aged Care, you will have 56 days to activate your service with your preferred provider.

If you choose Anglican Care as your provider, give us a call and speak to one of our HomeLife Care Partners. They will arrange a visit to your home and sit down with you to develop a care plan, taking into account everything that matters most to you.

Fees and contributions will depend on your funding level and personal circumstances – such as whether you are a full pensioner, part pensioner, or a self-funded retiree.

When meeting with our Care Partners, they will walk you through your individual circumstances and what it means for your care needs.
Yes, per the Australian Government’s guidelines, 10% of all service costs are dedicated to care management fees to ensure you get the support you need to coordinate and manage your care.
To access Home Support services, you will need to apply for Support at Home funding through My Aged Care.

To access Day Support Centres and Meals Made Easy services, you will need to apply for the Commonwealth Home Support Programme (CHSP) funding through My Aged Care.
Yes, we do.

Our Day Support Centres support all seniors living in the community, inclusive of seniors with memory loss and dementia. With our diversional therapist, dementia care lead, and activity officers creating an all-inclusive day of activities.

We provide strategies and support to participants, their families, and carers through our onsite information meetings.
We can tailor a package of care and support that is right for you.

As a guide, Home Support services can provide the following types of services: Personal care, home care, transport to appointments, meal preparation, basic garden care, in-home respite, medication prompts, shopping assistance and social activities.
